ILLINOIS FOOD ALLERGY EMERGENCY ACTION PLAN AND TREATMENT AUTHORIZATION
- A Sports Physical is only required if your child will be participating in one of our school sports listed on our Athletic page. A Sports Physical is not required for the 6th grader. The Medical Exam necessary for the 6th graders fulfills this requirement
